Clinical Policy Writer/Administrator (Nursing Home) Wokingham
Working with the Registered Manager and the Optalis Governance Team this role will support to help prepare clinical policies that ensure our residents receive the highest level of care.
The role will include creating approximately 35/40 policies, such as resuscitation, stroke, pain management, diabetes, catheter, immunization, clinical governance in care homes with nursing, and more. To give an indication of the type of policies we need.
Using Croner-i, we have a base policy template for many - but they need more clinical guidance/ instructional and be suitable specifically for Optalis and fit for purpose.

Service Manager – Maternity Cover
The Short-term support team is a well-established and valued community-based rehabilitation team, providing short term interventions of up to 6 weeks, to promote independence to residents in their own homes across the Royal Borough of Windsor & Maidenhead.
The service is multidisciplinary, including a team of Rehabilitation Care Assistants, Duty Officers, Occupational Therapists, Physiotherapists, and Team Coordinators to provide person centred support, helping to achieve agreed goals to maximize independence.

Effective Rota management, supervisions, and appraisals. The interviewing and development of new employees making sure that the service remains’ safe and ready for CQC inspection.

Main Duties and Responsibilities:
· Manage the delivery of safe and fully trained staff in person-centered support to residents of RBWM in their own homes.

Full Time Duty officer working with the STS&R team
This is a multi-disciplinary team providing short term, intensive rehabilitation and end of life support for up to six weeks in a care at home environment.The team comprises of occupational therapists, physiotherapists and rehabilitation assistants working in the community to support residents to achieve or regain independence.The service will assist to facilitate earlier discharge from hospital and prevent hospital admissions.The service works closely with health partners to ensure the best outcomes for the service user.
To take, triage and process referrals into the service.To manage and monitor a dispersed workforce during office hours.To allocate and co-ordinate the work of Rehabilitation Care Assistants delivering service in the community during core business hours.To support the duty telephone system during office hours.To process timely discharges from hospital , arrange additional support from partner agencies where required , facilitate delivery of equipment requested by qualified professionals.
The post holder will ensure that all front line staff are promoting Independence and choice for Service users in working towards their agreed outcome focused rehabilitation goals.• The post holder is expected to work from 8.45am to 5.15pm in the registered office .• Be responsible for the co-ordination and allocation of work schedules for the front line support staff team using a bespoke electronic scheduling system.• The post holder will co-ordinate daily service delivery to Service Users in line with a person centred support plan agreed with the service user and allied professionals.• Be responsible for recording and transferring information received and actions taken on shift using agreed procedures and systems. This involves daily use of Microsoft Word and Outlook in addition to bespoke database.• Be aware of budgetary implications, however no direct budgetary responsibility.• The post holder works in a constantly changing work environment which requires prioritisation of tasks to meet key deadlines.• Attend monthly team meetings on a rota basis to ensure knowledge is up to date.• Liaise with line manager, health and social care partners, Service Users and their families regarding changes in circumstances /level of need.Participate in the induction process of new team members by mentoring and through direct observation.• Be flexible to support the changing needs of the service and the customers.
Support Worker - Homeside, Maidenhead
Homeside is an 8 bedded residential care home registered with the CQC that provides 24 hour personal and practical support to people with learning disabilities within a safe and caring environment.
Main duties will include :
• Supervise and assist with client’s personal care
• Supporting with and organising activities, outings and recreation for service users.
• Provide opportunities for clients to make decisions and choices which affect their lives.
• Assist in creating a safe, homely and supportive environment
• To take on the role of key worker for approximately 2 residents.
• Assist in compiling care plans, risk assessments, report books and client records when required.
• To ensure the safe and appropriate running of the home in the absence of Managers and to inform the on-call Manager immediately of any concerns.
• Although there will usually be three people on duty throughout shifts the post holder may at times be left in sole charge of the building while their colleague assists residents with activities and appointments in the community.
• To undertake shift work (including weekends and bank holidays as needed)

The Statutory Team (Optalis) works in partnership with RBWM to support local residents.
Tasks involve Safeguarding sec42 of 2014 Care Act Legislation, Care Act assessment 2014, MCA 2005 and BI, Advice and Information, Tele Care Support, Liberty Protection Safeguarding DoLS, Support for Carers and Occupational Therapy support and Advocacy. Optalis provides a range of support and care services for older people, adults with a disability and people with mental health needs both in the community and at home.
To delivery Statutory Services to people who meet the eligibility criteria of the Adult and Community Services Directorate, by providing outcome focused assessments, and then facilitate the delivery of those identified services in order to achieve desired outcomes to meet the needs of the adult.
